Our Care Management Services For Seniors
Care Counselors provide the following care management services & solutions:
- Professional care assessments and consultations with the family to identify problems and develop an individualized plan of care
- Coordinate medical and other services needed to maintain safety and well-being
- Monitor and implement needed changes in care
- Provide short or long-term assistance for caregivers living near or far away
- Act as a liaison to families at a distance, overseeing day to day care, and quickly alerting families to problems
- Ongoing advocate and point of contact with physicians and other health care professionals
- Assist with public benefit applications such as nursing home Medicaid and SSI
- Provide court appointed Conservatorship services
- Review financial, legal or medical issues and offer referrals to geriatric specialists
- Assist with the management of medical, insurance and household bills
- Assist with moving Seniors to or from a retirement complex, assisted living or nursing home, when appropriate
- Provide geriatric care education and advocacy
- Additional services and support for Seniors seeking assistance for their adult disabled children
- Decision-making support
- Provide emotional support, guidance and advocacy during time of crisis and available 24 hours a day, 365 days a year…we’re here when you need us
